Medical Programs & Services
 

The York Region District Stroke Centre is part of the Medicine Program.  We are dedicated to those who require rehabilitation after suffering a stroke. Length of stay varies depending on our patients needs (generally 2 to 12 weeks). The focus of the service is to assist our patients to reach their optimum level of function and facilitate re-entry into the community safely. The stroke service is offered in two areas, Rehabilitation and Complex Continuing Care.

Our Philosophy of Care
Our primary goal is to maximize your strengths and abilities and to promote your independence. Your goals drive the process and are supported by our therapy team.

Who We Serve:

  • Patients diagnosed with having had a stroke.
  • Stable patients who are medically able to participate in active daily therapy.
  • Patients must have realistic, definable goals.
  • Patients must demonstrate motivation to improve and willingness to participate.

Discharge Guidelines
At admission, the patient must have definite and clear discharge arrangements. Plans are continually assessed in preparation for discharge. When you have progressed to a level that allows you to safely re-enter into the community to continue your rehabilitation, you will be discharged.
