About the Service
The Psychogeriatric Assessment Service is a comprehensive assessment and consultation service provided to seniors 65 years of age and older who are experiencing mental health difficulties and psychiatric symptoms.
Outreach and Assessment
The Psychogeriatric Assessment Service works in collaboration with the Integrated Psychogeriatric Outreach Program and Geriatric Outreach team |
Prior to the assessment by a Psychiatrist one of the outreach teams will conduct a visit to the client’s home. The home visit consists of a complete assessment including family history, medical history, mental health and social history. Any changes in behaviour, mood, general health that have occurred recently are recorded.
This is followed up by a consultation with a Psychiatrist at York Central Hospital. The consultation report and recommendation are then forwarded to the referral source.
Short term follow-up services are offered as indicated.

The Psychogeriatric Assessment Service accepts physician referrals only. Physicians may make a referral by calling 905-883 1212ext. 3361, or faxing completed Referral forms to 905-883 2139.
